Employment and Community First CHOICES program is administered by TennCare through its contracted managed care organizations. As an ECF provider we offer services to people with intellectual and developmental disabilities. Services in the program will help people become employed and live as independently as possible in the community. All new enrollment is in the Employment and Community First CHOICES program, as DIDD’s waivers are closed to new enrollment.
- Personal Assistance – We provide the necessary supports for clients to live in the community. Personal assistance services are geared more toward hands-on activities, such as bathing, dressing, toileting needs and transferring between bed and wheelchair or wheelchair and car etc. Other services include cleaning the home, shopping, running errands etc., while enhancing opportunities for those we serve.
- Community Integration Support Services – Ensuring self-determination, independence, empowerment, integration and inclusion of all people with disabilities in all parts of society. We strive to incorporate community activities and events that are person centered and provide opportunities for those we serve. Our desire is that all clients have support systems in place that include friends, family, natural supports, advocates etc.
- Independent Living Skills Training – Our goal is to provide leaning opportunities through everyday life experiences. We are goal oriented and work on areas that service recipients desire more independence in. Services are outcome based and monitored by all aspects of progression to move on to the next desired skill.
- Community Transportation – We provide transportation to all community outings, including medical and therapeutic appointments, errands; grocery shopping, paying monthly bills, etc. Any and all community activities that the client wishes to attend or participate in we provide community transportation.
- ECF CLS Level 1-8 – We provide support in a residential setting in the clients home. Supports provided are determined by the service recipients level of need required. Clients live with up to 2 roommates in a private home setting. This allows for additional support with meeting their monthly financial obligations. This also allows for additional support and friendships to be formed. We provide Med administration, medical oversight, assisting in all areas that are needed, including personal care, household chores, transportation to any and all appointments and community events. We provide all necessary supports needed to successfully allow clients to live as independent as possible.
