We provide Residential and Community Based services as contracted with Tennessee’s Department of Human Services and the Department of Intellectual and Developmental Disabilities (DIDD)
We strive to ensure that each person’s services are based on his or her specific needs and desires. We provide staff members that are well-trained and prepared for service through a combination of classroom instruction, online training, and client-specific in-services. This training is completed throughout the year and as needed Our staff works with each person’s team to provide supports that allow every individual to achieve and maintain their highest possible level of independence at home and in the community.
We Recognize the desires of our clients. We strive to identify people’s strengths and talents. We honor the role of family members, friends, and natural supports. We always utilize available community resources to assists the clients we service. As a team we problem-solve and find the best solutions and advocate for everyone we serve. We will always encourage self-advocacy while we strive to educate communities.
- Supported Living -Supports provided by well trained, oriented staff that provide supports in the clients home in a residential setting, with staff working in shifts to provide 24 hour services. We provide Med administration, medical oversight, assisting in all areas that are needed, including personal care, household chores, transportation to any and all appointments and community events. We provide all necessary supports needed to successfully allow clients to live as independent as possible.
- Semi-Independent Supported Living- Supports from well trained and oriented staff that provide needed supports in the clients personal home, that may include, but not limited to: meal preparation, transportation to medical appointments, general house hold chores, assisting in grocery shopping, paying monthly bills etc. All services are based on each clients desired interests and personal outcomes. Our desire is to provide and accommodate the highest level of independence for each client we serve.
- Community Based Day services – CB day is meant to be meaningful and to help build relationships within the community. All services focus toward community inclusion, with the goal of being a valued member of the community. All activities are person centered and reflect the individual’s preference. The expectation is that the person will spend 6 hours in the community participating in an integrated community setting. Emphasis on identifying and exploring different things to allow the benefits of daily recreational, therapeutic and socialization activities.
- In-Home Day Services-In home day services are based on criteria that prevents the client from being in the community on a particular day. Services are provided in the individual’s home, only when CB day is not an option. All services are goal oriented with focus on the clients’ interests, desires and personal outcomes to achieve.
